TENDER DESCRIPTION

Ophthalmic laser, compliant with the minimum parameters contained in Annex No. 2 to the SWZ. The Contracting Authority requires the delivery, as part of the contract, of factory-new devices and medical products. Offering used, refurbished/reconditioned and demonstration devices and medical products is not permitted. Devices and medical products must be free from all physical (structural) defects and legal defects. The proposed equipment must have the required certificates and markings provided for in the provisions of the Act on Medical Devices, indicating the required authorization for use and placing on the market in Poland, i.e. the CE certificate and the declaration of conformity in accordance with the Regulation of the Minister of Health on the essential requirements and conformity assessment procedures for medical devices, or equivalent ones resulting from the type and intended purpose. The offered item of the contract must be complete and ready for use without any additional purchases. Warranty: 60 months from the date of the final protocol acceptance without comments.
