1. The subject of the contract is the supply of equipment and devices for the provision of medical services at WOMP CPL – 8 parts. The contract has been divided into 8 parts: Part 1 – Set for exercise tests of the heart Part 2 – OCT device – optical coherent tomograph Part 3 – TECAR therapy device Part 4 – Laboratory centrifuge Part 5 – Hematology microscope Part 6 – Disinfector washer Part 7 – Rektoscope Part 8 – Operating table 2. As part of the performance of the contract, the Contractor undertakes to: a) deliver the medical equipment forming the subject of the contract together with the documents required in the SWZ to the Contracting Authority’s registered office, b) install the medical equipment at the location indicated by the Contracting Authority, c) install and start up the medical equipment and confirm the proper functioning of the supplied devices, d) provide training for the staff of the Contracting Authority in the operation and rational use/exploitation of the supplied medical equipment, e) deliver together with the medical equipment operating instructions in Polish in electronic and paper versions, warranty cards, technical documentation necessary for the…
