TENDER DESCRIPTION
1. The subject matter of the contract is the supply of single-use sterile medical devices for USK-2 PUM in Szczecin. 2. Whenever in the further part of the SWZ the term “devices” is used, it shall be understood to mean all devices listed in Appendix No. 2 (FCJ) to the SWZ. 3. The description of the subject matter of the contract has been included in the unit price form (Appendix 2 to the SWZ, hereinafter also referred to as “FCJ”) and must be submitted together with the tender form (Appendix 1 to the SWZ, hereinafter also referred to as “FO”). 4. The Contracting Authority requires that the offered medical devices—within the meaning of the Act of 7 April 2022 on medical devices (Journal of Laws 2024.1620, as amended)—are placed on the market and used in the territory of the Republic of Poland in accordance with applicable regulations. 5. The quantities of the devices provided in the unit price form are indicative in nature and the Contracting Authority has the right to order smaller quantities, provided that they are not less than 60% of the contract/task value. 6.
