TENDER DESCRIPTION

1. The subject of the contract is the delivery of sterile disposable medical devices for USK-2 PUM in Szczecin. 2. Wherever in the further part of the Terms of Reference the term "devices" is used, it should be understood to mean all devices indicated in appendix no. 2 (Unit Price Form) to the Terms of Reference. 3. The description of the subject of the contract is included in the unit price form (appendix 2 to the Terms of Reference, hereinafter also referred to as "Unit Price Form") and must be submitted together with the offer form (appendix 1 to the Terms of Reference, hereinafter also referred to as "Offer Form"). 4. The Contracting Authority requires the offered medical devices - within the meaning of the Act of 7 April 2022 on medical devices (Journal of Laws 2024, item 1620, as amended) - to be introduced to the market and used on the territory of the Republic of Poland in accordance with applicable regulations. 5. The quantities of devices given in the unit price form are indicative and the Contracting Authority has the right to order smaller quantities, provided that not less than 60% of the contract/task value. 6.
