1. The subject of the contract is the supply of construction materials (contract split into 2 parts). 2. The subject of the contract has been divided into 2 (two) parts: 1) Part No. 1: Supply of road slabs and pine conifer beams to the towns of Korczowa, Medyka, Stubno; 3. The following form an integral part of the description of the subject of the contract: 1) Price calculation form — Appendix No. 1A to the SWZ — this document also serves as the price calculation form and the description of the subject of the contract. 2) Draft contractual provisions — Appendix No. 4A. 4.
