TENDER DESCRIPTION

1. The subject of the contract is: Delivery of laboratory reagents and diagnostic tests for histopathological, microbiological and HLA diagnostics for USK-2 PUM in Szczecin 2. Whenever in the subsequent part of the SWZ the following term is used, it shall mean: a) “products” – this shall mean products and consumable materials specified in Annex No. 2 (FCJ) to the SWZ; b) “equipment” – this shall mean the rented apparatus for reading the results from the medium for testing the microbiological cleanliness of surfaces – applies to task no. 7. 3. The description of the subject of the contract and the estimated quantities for the products are included in the unit price form (Annex 2 to the SWZ, hereinafter also as FCJ and Annex 2a of the OPZ for task no. 5). The unit price form, Annex 2 to the SWZ, must be submitted without fail together with the tender form (Annex 1 to the SWZ, also referred to hereinafter as “FO”). 4.
