TENDER DESCRIPTION

Supply of urine test strips and reagents for washing liquids and control materials together with the lease of an automatic analyser for the Healthcare Team No. 2 in Rzeszów 1. The subject of the contract is the successive supply of urine test strips and reagents for washing liquids and control materials together with the lease of an automatic analyser for the physico-chemical analysis and formed elements of urine for the Medical Diagnostic Laboratory of the Healthcare Team No. 2 in Rzeszów for a period of 36 months. The detailed list of products constituting the subject of the contract and their quantities is contained in Annex No. 1 to the SWZ entitled “Assortment-and-price form” and in the Description of the subject of the contract, which constitutes Annex No. 2 to the SWZ. 2. The list of the subject of the contract contained in Annex No. 1 and Annex No. 2 to the SWZ forms an integral part of this specification. The full assortment listed in the Annexes to the SWZ is required. The absence of any assortment from any of the Annexes will result in rejection of the tender.
