TENDER DESCRIPTION

Evidence documents: 1) In the case of medical devices – attaching current and valid marketing authorization documents valid throughout the term of the contract for each offered product (in the form of a Declaration of Conformity issued by the manufacturer / CE Certificate issued by a notified body, Notification/Report Form to the President of the Office (in accordance with art. 138 of the Act of 9 May 2022 on medical devices)) 2) In the case of medicinal products – attaching current and valid marketing authorization certificates valid throughout the term of the contract for each offered medicinal product in accordance with the Act of 6 September 2001 Pharmaceutical Law and SPC. 3) In the case of cosmetic products – attaching current and valid documents throughout the term of the contract confirming notification to the Cosmetic Product Notification Portal (CPNP) in accordance with the Act of 4 October 2018 on cosmetic products. 4) In the case of biocidal products – attaching valid and current documents throughout the term of the contract indicating approval for placing on the market in the territory of the Republic of Poland for the offered biocidal product - pursuant to the Act…
