TENDER DESCRIPTION

1. The subject of the contract is the delivery of various medical products, including implants used in the treatment of hearing loss, arthroscopic implants, products used in neurosurgical procedures for Copernicus PL Sp. z o.o. 2. A detailed description of the subject matter of the contract specifying the requirements of the Contracting Authority and estimated quantities is included in Appendix No. 1 to the SWZ – the assortment-and-price form. By filling in the positions indicated in Appendix No. 1 to the SWZ, the Contractor confirms compliance with the requirements specified by the Contracting Authority. 3. The Contractor is required, for the first delivery, to provide a list of medical equipment/instruments loaned for the duration of the agreement, or, alternatively, for each delivery, to provide a list of the medical equipment/instruments loaned for the procedure, in accordance with Appendix No. 1A to the SWZ – if applicable. 4.
