TENDER DESCRIPTION

1. The subject of the contract is the supply of pharmaceutical raw materials for the preparation of prescription compounded medicines for the purposes of the Prescription Laboratory of the Hospital Pharmacy of USK-2 PUM in Szczecin, hereinafter also referred to as “products”. 2. The description of the subject matter of the contract and the estimated quantities relating to the products are included in the unit price form (Annex 2 to the SWZ, hereinafter also referred to as FCJ). 3. For item No 27, the Contracting Authority requires the offer of medicinal products in accordance with the Act of 6 September 2001 Pharmaceutical Law (Journal of Laws 2001.126.1381, as amended), placed on the market and used in the territory of the Republic of Poland in accordance with the applicable regulations. 4. The Contracting Authority requires the Contractor to offer: a) pharmaceutical raw materials used to prepare prescription compounded medicines; b) finished medicinal products within the scope of item 27, which may also be used as prescription raw materials; c) auxiliary products of items 50-51. 5.
