TENDER DESCRIPTION

1. The subject of the contract is the supply of radiotherapy devices, including thermoplastic masks, masks and headrests for immobilising the patient, vacuum mattresses, and boluses for the Clinical Department of Radiotherapy of the University Clinical Hospital in Rzeszów, ul. Szopena 2, including: Boluses: silicone, individual, thermoplastic. 2. A detailed description of the items covered by the contract, the estimated quantity, and the requirements of the Contracting Authority have been specified in the Price Form – Annex No 1 to the SWZ for Part No 1, which forms an integral part of the SWZ. Information regarding the rules for performing deliveries is contained in Annex No 6 to the SWZ – Draft provisions of the contract for a public procurement. 3. The scope of the contract includes the delivery of the items covered by the contract in quantities specified in the Price Form Part No 1 (Annex No 1 to the SWZ) for a period of 24 months from the date of conclusion of the contract. 4.
