TENDER DESCRIPTION

The subject of the contract is the supply of single-use medical devices for the needs of the Pulmonology and Thoracic Surgery Center in Bystra within Package no. 1 - Closed blood collection system using aspiration-vacuum or vacuum method. A detailed description of the subject of the contract in terms of assortment and quantities is specified in the assortment and price form - Annex no. 2 to the ToR. A full scope offer for the subject of the contract within the Package is required. The expiry date of the offered assortment must be at least 6 months from the moment of delivery of the order to the Contracting Authority, unless another expiry date is indicated in Annex no. 2 to the ToR. The Contracting Authority requires all items to be new and unused. All offered medical materials classified as medical devices must be introduced into trade and use on the territory of the Republic of Poland, classified according to applicable regulations: EU Regulation 2017/745 on medical devices, EU Regulation 2017/746 on in vitro diagnostic medical devices, Act of 7 April 2022 on medical devices (consolidated text: Journal of Laws 2024, item 1620), as applicable.
