TENDER DESCRIPTION

Sought in this seventh competition of the contracting entities are: care solutions that can be represented within the framework of care contracts or software license agreements with a close care reference according to SGB V and/or XI with health and/or nursing care funds. The sought care solutions can be hybrid, combining classical and digital care, or represent purely digital solutions. Further requirements for the sought care solutions: - Proof of a well-thought-out and comprehensible care pathway or a well-thought-out and comprehensible overall care concept ("patient journey"), in which the care solution is, for example, embedded in care processes within the framework of a care contract possibly involving other service providers and/or presented across sectors. - If it is necessary to involve third parties (e.g., certain service provider groups or management companies) in the care solution, it should be demonstrated how binding existing contacts with these third parties are.
