TENDER DESCRIPTION

The subject of the contract is the supply of orthopedic implants, in accordance with Appendix No. 2 to the SWZ, i.e. the item-and-price schedule. The detailed terms for performance and settlement of the subject of the contract are specified in the contract template constituting Appendix No. 10 to the SWZ. Due to the software used by the Contracting Authority, any interference with the item-and-price form is prohibited. The Contracting Authority requests that the Item-and-price Forms be submitted together with the offer in the format .xls, .xlsx or in another spreadsheet format. Changes are not permitted by adding columns, removing columns, removing the spreadsheets of individual parts, and others.
