RENTAL OF AN APPARATUS FOR CONTINUOUS RENAL REPLACEMENT TECHNIQUES FOR A PERIOD OF 12 MONTHS 1. The subject of the contract is a service to rent an apparatus for performing continuous renal replacement techniques in the quantity of 1 unit, in accordance with the description and requirements set out in Appendix No 1 to this SWZ. 2. The boundary parameters regarding the apparatus are described in Appendix No 1 to this SWZ. 3. Additional conditions concerning the subject of the contract: a) The Contractor shall grant the Contracting Authority a warranty for the duration of the apparatus rental, for a minimum period of 12 months from the date of signing the contract; b) The Contractor shall bear all costs related to maintaining the rental subject matter (e.g. technical inspections in accordance with the manufacturer’s recommendations, ongoing repairs, periodic and service inspections, and maintenance of the apparatus including consumable materials) during the term of the contract; c) The Contractor shall provide the Contracting Authority with contact assistance 24 hours a day, 7 days a week; d) the required service response time to a fault report via telephone contact with the…
