1. The subject of the contract is: Service of medical equipment at the Client’s location at 1 Powstania Styczniowego Street, 81-519 Gdynia. 2. A detailed description of the subject matter of the contract, assortment and estimated quantities are provided in Annex No. 2 to the SWZ – Assortment-and-Price Form, Annex No. 7 to the SWZ – Description of the subject matter of the contract and the scope of required servicing activities, and Annex No. 5 to the SWZ – Draft contract. 1.
