TENDER DESCRIPTION

1. The subject of the contract is the supply of disposable medical materials and products to the 115th Military Hospital SP ZOZ in Hel in division into 12 lots: Lot 1 - Surgical sutures. 2. A detailed description of the subject of the contract, the assortment and estimated quantities, as well as the required parameters and boundary conditions, is described in Appendix No. 2 to the SWZ – the assortment-and-price form. 3. The medical products offered by the Contractor must, in terms of quality, meet the requirements for medical devices permitted for circulation and use in the territory of the Republic of Poland in accordance with the Act on Medical Devices of 7 April 2022 (consolidated text: Journal of Laws of 2024, item 1620). 4. The single commercial unit packaging of the subject of the contract must contain (in the Polish language): the name of the product, the quantity in the package, the type of material, the manufacturer, the series, the expiry date, and the CE mark. 5.
