TENDER DESCRIPTION

The subject of the contract is: Successive deliveries of medicinal products. A detailed description of the subject matter of the contract has been indicated in Appendix No 2 to the SWZ - price-and-technical form for Task No 1 1. The Contractor declares that the medicinal product offered under this task has valid documents permitting circulation on the territory of the Republic of Poland - in accordance with applicable law. At the stage of contract performance, copies of the above documents and the medicinal product characteristics will be provided to the Contracting Authority immediately upon its request. 2. The Contracting Authority requires that the proposed product name together with the EAN code (column No 10) be obligatorily provided. 3. The Contracting Authority allows the pricing of preparations in packages of a different size than that provided by the Contracting Authority, and their quantity must be appropriately recalculated by rounding up. 4. A medicine that is equivalent in terms of chemical composition and dosage but differs in form, while maintaining the same route of administration, may be replaced within the following scope: tabl. powl.-tabl.; tabl.-coated…
