TENDER DESCRIPTION

Provision of physiotherapy services for residents of the Paradyż commune 1. The subject of the contract is a service consisting in carrying out individual physiotherapy for residents of the Paradyż commune who have a referral from a doctor directing them to treatment procedures. 2. The service will consist of individual therapy for each resident of the Paradyż commune. 3. The tasks will be carried out from Monday to Friday, 7.5 hours per day. 4. Physiotherapy procedures shall be performed by 2 persons who have: a. the appropriate qualifications (entitlements) of a physiotherapist in accordance with the Act of 25 September 2015 on the profession of physiotherapist; b. an entry in the National Register of Physiotherapists; c. at least 1 person with one-year experience in the profession of physiotherapist. 5. Employment conditions: self-employment. 6. During the provision of physiotherapy treatments for patients, the Contractor will use free of charge: a) premises at ul.
