TENDER DESCRIPTION

1. The subject of the contract is the provision of individual psychotherapeutic support for participants (hereinafter: UP) of the “It’s about the individual” project within the area of the Gmina Klucze. The service will be addressed to up to 6 UP who, on the basis of individual diagnosis of needs and potential prepared by 4 specialists, require coverage in this form of support. 2. The aim of the service is to mitigate or eliminate health barriers that hinder functioning in society. 3. The outcome of the support provided should be an understanding of oneself and more effective coping with emotional difficulties, conflicts and relationships, as well as improvement in the area of emotional well-being and reduction of stress. 4. Under the subject matter of the contract, it is planned to provide a maximum of 174 hours. 5. The provision of the service will take place from the date of signing the contract until 30.04.2027. 6. Activities forming the subject matter of the contract will be carried out at the Centre for Social Services in Klucze, at ul. Zawierciańska 16, ul.
