Provision of medical services for employees of the Office of the Financial Ombudsman 1 The subject of the contract includes the provision of medical services for the Office of the Financial Ombudsman, covering the following packages: 1.1 Employees’ Package, basic variant, 1.2 Employees’ Package, extended variant, 1.3 Partners’ Package, basic variant, 1.4 Partners’ Package, extended variant, 1.5 Family Package, basic variant, 1.6 Family Package, extended variant, 1.7 Seniors’ Package, 1.8 Occupational Medicine Package (a service for issuing a single medical certificate together with the necessary tests, consultations). 2 Under the option referred to in Article 441 of the Public Procurement Law (hereinafter “the Option” or “optional contract”), the Contracting Authority reserves the right to extend the performance of the contract, for a maximum of a further period of 12 months, with the extension being made multiple times, each time for a period not shorter than 1 month. 3 The Contracting Authority reserves the right not to implement the Option in full or in part.
