TENDER DESCRIPTION

The subject of the contract is the provision of occupational medicine services for employees of the Ministry of National Defence and for soldiers of the 22nd Military Supply Unit in Olsztyn, as well as for units and institutions supplied by it (pre-employment, periodic, and follow-up examinations) and other medical examinations. The method of performance of the contract has been specified in the draft contract provisions, constituting Appendix No 6 to the SWZ. Pursuant to Article 441 of the Act of 11 September 2019 – Public Procurement Law, the Contracting Authority provides for the possibility of exercising its option rights with respect to each of the parts into which this contract has been divided, in accordance with the provisions of the draft contract provisions. Option right – provision of medical services within the scope of the contract subject, upon the Contracting Authority’s written notification, in the quantities indicated by it within the contract performance period and at the unit prices specified in the price form.
