PROVISION OF SERVICES FOR THE COLLECTION, TRANSPORT AND DISPOSAL OF MEDICAL AND HAZARDOUS WASTE 1. The subject of the contract is the provision of services for the collection, transport and disposal of medical and hazardous waste in the quantity and assortment specified in the Pricing Form – Appendix No 2 to the SWZ. 2. Services related to the collection, transport and disposal of medical waste marked with code 18 01 and waste with code 15 02 are designated in accordance with the Regulation of the Minister of Climate of 2 January 2020 on the waste catalogue (Journal of Laws of 2020, item 10). 3. The manner of performance of the service and the Contractor’s obligations are specified in detail by: 1) Description of the subject of the contract constituting Appendix No 3 to the SWZ – “hereinafter referred to as the OPZ” 2) Draft contract constituting Appendix No 6 to the SWZ. 4.
