TENDER DESCRIPTION
1. The subject of this procurement procedure is the service of transporting patients by medical transport, transporting a doctor for the collection of biological material, transporting samples for testing and/or transporting a transplant team together with the equipment to the organ collection site for transplantation for the University Clinical Hospital No. 1 named after N. Barlicki in Łódź (hereinafter referred to as the service), in accordance with the types and quantities specified in the “Assortment-and-Price Form” constituting Annex No. 2 to the SWZ and in accordance with the Tender Form constituting Annex No. 1 to the SWZ, and the Detailed Description of the Subject of the Contract constituting Annex No. 1a and 2 to the SWZ.
