1. The subject of the proceedings is: Group life insurance service for employees and family members of employees of the Voivodeship Sanitary and Epidemiological Station in Rzeszów together with the District Sanitary and Epidemiological Stations of the Podkarpackie Voivodeship. The contracting authority does not guarantee that all employed employees and family members of employees 2. of the contracting authority will join the group life insurance. During the insurance period, changes in the number of insured persons may occur. If an employee or a family member of an employee resigns from insurance, the contracting authority is not obliged to pay the insurance premium for them. 3.
