1. The subject of the contract is: a Group life insurance service for employees and members of employees’ families of the Independent Public Healthcare Facilities Team in Ostrowa Mazowiecka. 2. The Contracting Authority does not guarantee that all employed employees and members of employees’ families of the Contracting Authority will take part in the group life insurance. 3. During the term of the insurance period, changes may occur in the number of insured persons. 4. If an employee or a member of an employee’s family resigns from the insurance, the Contracting Authority is not obliged to pay the insurance premium for them. 1.
