TENDER DESCRIPTION

1. The subject of the contract is health transport services for patients, transport of biological materials, and transport of blood and its components for the University Clinical Hospital named after Fryderyk Chopin in Rzeszów at Szopena Street 2 and at Rycerska Street 2. 2. A detailed description of the subject matter of the contract is included in the Description of the subject matter of the contract (Appendix No 2 to the SWZ) and in the Price–Offer Form (Appendix No 1 to the SWZ), which constitute an integral part of the SWZ. 3. The scope of the contract includes provision of services for the above-listed items for a period of 36 months. 4. The Contracting Authority allows submission of equivalent offers. 5. CPC category: 2. 6. CONTRACT PERFORMANCE DEADLINE: Contract performance period: for 36 months, starting from the date of conclusion of the contract.
