Support for You - development of deinstitutionalised social services - Provision of specialist care services in the Municipality of Malanów Description of the subject matter of the contract - implementation of specialist care servicesSpecialist care services - will be provided in the Municipality of Malanów for residents of the Municipality of Malanów at the place of residence in the period from 01 September 2026 to 30 September 2028. The task will be carried out in accordance with Article 50 of the Act of 12 March 2004 on social assistance (Journal of Laws of 2026, item 639) and the Regulation of the Minister of Social Policy of 22 September 2005 on specialist care services (Journal of Laws of 2024, item 816).The scope of the services will be tailored to the individual needs of persons requiring assistance in the form of specialist care services, resulting from the type of their ailment or disability, including the following scope:a) care as support for the treatment process, including:- assistance in accessing health care services;- agreeing on and monitoring the dates of doctors’ visits and diagnostic tests;- assistance in purchasing/ordering medicines at the pharmacy;-…
