TENDER DESCRIPTION

1) The subject of the contract is the equipment for the endoscopy laboratory. 2) The subject of the contract is described in detail in Annex No. 1 to the ToR, the assortment and price form (Description of the subject of the contract - OPZ). 3) Detailed terms and conditions concerning the execution of the subject of the contract are regulated in the Draft Agreement, Annex No. 4 to the ToR (Contractual Conditions). 4) The Contracting Authority requires the Contractor to indicate in the offer the parts of the contract which it intends to subcontract and to provide the names of the subcontractors – in accordance with Article 462(2) of the Public Procurement Law, if already known. 5) If a change or resignation from a subcontractor concerns an entity whose resources the Contractor relied on, under the principles laid down in Article 118(1) of the Public Procurement Law, to demonstrate compliance with the participation conditions, the Contractor is obliged to demonstrate to the Contracting Authority that the proposed other subcontractor or the Contractor itself independently meets them to no lesser extent than the subcontractor whose resources the Contractor had relied on during the…
