TENDER DESCRIPTION

1. The subject of the contract is the purchase of an extracorporeal circulation apparatus for Copernicus PL Sp. z o.o. in Gdańsk. 2. The detailed scope and description of the subject of the contract, estimated quantities and the manner of performing the contract are set out in the attachments to the Terms of Procurement (hereinafter referred to as SWZ), namely: 1) Annex No. 1a to the SWZ – Price Form, 2) Annex No. 1b to the SWZ – Description of the Subject of the Contract, 3) Annex No. 3 to the SWZ – Draft contract. 3. The detailed description of the subject of the contract specifying the requirements of the Contracting Authority and quantities is contained in Annex No. 1b to the SWZ – Description of the Subject of the Contract. By completing the indicated items in this attachment, the Contractor confirms compliance with the requirements specified by the Contracting Authority. 4. The manner of performing the subject of the contract is specified in the draft contract – Annex No. 3 to the SWZ - single delivery. 1.
