“Purchase of an ultrasound (USG) device for COPERNICUS PL Sp. z o.o. in Gdańsk” 1. The subject of the contract is the purchase of an ultrasound (USG) device for COPERNICUS PL Sp. z o.o. in Gdańsk. 2. The subject of the contract consists of 1 part. The detailed description of the subject of the contract specifying the requirements of the Contracting Authority and quantities includes the following appendices:  Appendix to the SWZ – Form of Tender,  Appendix No. 1 to the SWZ – Price Form,  Appendix No. 2 to the SWZ – Description of the Subject of the Contract,  Appendix No. 3 to the SWZ – Draft Contract together with its appendices. Appendices No. 1 and No. 2 to the SWZ the Contractor attaches to the tender. By filling in the indicated items in Appendix No. 2 to the SWZ – Description of the Subject of the Contract, the Contractor confirms compliance with the requirements specified by the Contracting Authority. The scope and manner of performance of the subject of the contract are defined in Appendix No. 3 to the SWZ – Draft Contract together with its appendices.
