TENDER DESCRIPTION
1. The subject of the contract is the successive delivery of psychotropic medicines, narcotics, and low-molecular-weight heparins, divided into 21 parts, with delivery to three UCK WUM locations: - UCK WUM CSK pharmacies, at Nielubowicza St 5, 02-097 Warsaw - UCK WUM DSK pharmacies, at Żwirki i Wigury St 63a, 02-091 Warsaw - UCK WUM SKDJ pharmacies, at Lindleya St 4, 02-005 Warsaw 2. A detailed description of the subject matter of the contract is set out in Annex No 2 to the SWZ - the assortment-and-price form (respectively within the scope of the Part to which it applies). 1.
