TENDER DESCRIPTION

1. The subject of the contract is the purchase and delivery of a computed tomography scanner in the quantity of 1 unit, together with equipment, transport, carrying-in, performance of the necessary adaptation works of the premises, installation, commissioning and the performance of staff training for the University Clinical Hospital named after Fryderyk Chopin in Rzeszów. 2. The detailed description of the subject of the contract and the technical and functional requirements relating to Part No 1 have been specified in the Description of the Subject of the Contract (OPZ), constituting Annex No 1 to the SWZ. The price offer form (FCO) for Part No 1, constituting Annex No 2 to the SWZ, specifies the scope subject to valuation, including the required number of units. Annex No 3 to the SWZ – “Minimum scope of adaptation works” – specifies the minimum scope of works necessary to perform the subject of the contract. The draft provisions of the contract, constituting Annex No 11 to the SWZ, specify the rules for performing the subject of the contract. The indicated documents form an integral part of the SWZ. 3.
