1. The subject of the contract is the purchase of medical equipment and apparatus for Hospital Wards and Operating Theaters of the Wojewódzki Szpital Specjalistyczny No. 4 in Bytom, Al. Legionów 10, as part of the implementation of investment tasks co-financed from the funds of the Śląskie Voivodeship. 2. The detailed description of the subject matter of the contract is specified in Appendix No. 1A – Assortment and Pricing Form, Appendix No. 1B – List of parameters and required conditions.
