THE FOLLOWING MUST BE ATTACHED TO THE TENDER: 1. Appendix No. 1 to the SWZ — tender form (completed and signed) in electronic form, provided with a qualified electronic signature, 2. Appendix No. 2 to the SWZ — statement on non-coverage by sanctions, resulting from the EU regulation concerning Article 5k of Regulation 833/2014 (completed and signed) in electronic form, provided with a qualified electronic signature, 3. Appendix No. 3 to the SWZ — ESPD (JEDZ) (completed in the scope indicated by the Contracting Authority, valid as of the date of tender submission and signed) in electronic form, provided with a qualified electronic signature, 4.
