TENDER DESCRIPTION

The subject matter of the contract is the sale, including delivery, of equipment. A detailed description of the subject matter of the contract is included in Appendix A to the SWZ. For each item, the Contractor shall indicate in the assortment-and-price form (Appendix No. 2 to the SWZ) the name of the Manufacturer / Importer and the manufacturer’s code. An attachment forming an integral part of the future contract will be the unit price form indicating the items and their unit prices. The Contractor shall attach to the tender the following evidence measures: a) A technical description of the proposed subject matter containing all necessary information enabling the assessment of the tender’s compliance with the description of the subject matter of the contract - Appendix A to the SWZ (i.e., in particular catalog sheets of the offered devices, information folders, a list specifying the manufacturer and model of the offered devices, configurations and parameters in the Polish language, etc.).
